Background:
Evaluation of hypothalamic-pituitary-adrenal axis

For failed synacthen tests, ACTH can help differentiate primary adrenal insufficiency from pituitary disease.
In cases of non-suppression of cortisol post-dexamethasone, ACTH can help differentiate Cushing's due to adrenal cortisol secretion from pituitary or ectopic cause.
